4- BLDG MUST BE FULLY INSULATED UNDER THIS SHELL PERMIT AS REQ'D.
5- PENDING SUBMITTAL OF SIGNED NOTIFICATION FROM THE PROFESSIONAL STRUCTURAL ENGR OF RECORD ATTESTING DESIGN RESPONSIBILITIES FOR REVIEWS AND COORDINATION OF ALL STRUCTURAL PARTS IN THIS PROJECT (WHETHER OR NOT DESIGNED BY OTHERS).
6- PENDING SUBMITTAL OF STATEMENT OF CODE COMPLIACE FORM SIGNED AND SEALED IN FULL AS REQ'D.
